Rafael Jeronym Kubelik (29/06/1914 - 11/08/1996), a Bohemian-born (Bychory, Bohemia), Swiss conductor and composer who was known for his powerful and invigorating interpretations, especially of the music of composers from his homeland. He made frequent guest appearances with major orchestras throughout the world. He led the Czech Philharmonic from 1941 to 1948 and the Chicago Symphony from 1050-1953. He was then musical director at London's Covent Garden opera house and from 1961-1979 headed the Munich orchestra of Bavarian Radio. He was the son of Czech violinist Jan Kubelik.

Requiem pro memoria uxoris is for bariton, boys choir, mixed choir and orchestra. According to the title, this requiem might be dedicated to the composer's wife. His Cantata pro memoria patris (1940/1941) might be a sort of requiem, as well.
